Paediatric Acute care Guidelines PMH Emergency Department

This guideline outlines the process of transferring patients who require non tertiary paediatric admission from the PMH ED to metropolitan hospital paediatric wards. The aim is to where possible enable children to be cared for closer to home.

The six metropolitan area hospitals incorporated in this guideline are:

  • Armadale Kelmscott Memorial Hospital (AKMH)
  • Fiona Stanley Hospital (FSH)
  • Joondalup Health Campus (JHC)
  • Peel Health Campus (PHC)
  • Rockingham General Hospital (RGH)
  • Midland Health Campus (MHC)

The following outlines the general approach from decision to admit in ED to arrival to the general hospital inpatient paediatric ward 

  • Decision to Admit
  • Patient medically appropriate for transfer to general hospital
  • Discuss transfer with parent/carer
  • Flag general hospital by postcode/suburb –  Local hospital catchment areas
  • Discuss with PMH ED coordinator/navigator regarding bed availability (if known)
  • Follow the specific process for each of the receiving metropolitan hospitals as below

Default to the normal PMH admission process if:

  • Parents decline transfer
  • Bed availability or patient acceptance is not achieved
    • Contact the PMH Discharge Coordinator on 0416 569 702 or page: 7240 to flag the potential for future transfer of the patient to their local  hospital. 
Armadale Kelmscott Memorial Hospital (AKMH)
Medical 
  • Contact the Hospital Clinical Manager on 0414 276 425 to confirm bed availability
  • Contact AKMH on call Consultant to arrange acceptance and transfer of care
  • Inform PMH ED Coordinator of confirmed transfer
  • Provide all patient notes to Clerk to copy for transfer
  • Complete EBM  

Nursing

  • Arrange appropriate transport (e.g. private, transport ambulance, paramedic ambulance)
  • Contact AKMH Children’s Ward on (08) 93341234 to give nursing handover and provide ETA 
Fiona Stanley Hospital (FSH)

Medical

  • Contact FSH Children’s Ward (Ward 3A) Nursing coordinator on (08) 6152 9485 to confirm bed availability
  • Contact FSH on call Consultant to arrange acceptance and transfer of care (after 2200hrs, must be Consultant to Consultant handover)
  • Inform PMH ED Coordinator of confirmed transfer
  • Provide all patient notes to Clerk to copy for transfer
  • Complete EBM

Nursing

  • Arrange appropriate transport (e.g. private, transport ambulance, paramedic ambulance)
  • Contact FSH Children’s Ward Nursing coordinator on (08) 6152 9485 to give nursing handover and provide ETA 
Joondalup Health Campus (JHC)

Medical

  • Contact JHC Children’s Ward Nursing coordinator on (08) 9400 9300 to confirm bed availability
  • Contact JHC on call Consultant to arrange acceptance and transfer of care
  • Inform PMH ED Coordinator of confirmed transfer
  • Provide all patient notes to Clerk to copy for transfer

Nursing

  •  Arrange appropriate transport (e.g. private, transport ambulance, paramedic ambulance)
  • Contact JHC Children’s Ward Nursing coordinator on (08) 9400 9300 to give nursing handover and provide ETA 
Peel Health Campus
 Medical
  • Contact PHC Children’s Ward Nursing coordinator on (08) 9583 6211 to confirm bed availability
  • Contact PHC on call Consultant to arrange acceptance and transfer of care
  • Inform PMH ED Coordinator of confirmed transfer
  • Provide all patient notes to Clerk to copy for transfer

Nursing

  • Arrange appropriate transport (e.g. private, transport ambulance, paramedic ambulance)
  • Contact PHC Children’s Ward Nursing coordinator on (08) 9583 6211 to give nursing handover and provide ETA 
Rockingham Kwinana Health Service
 Medical 
  • Contact RGH Children’s Ward Nursing coordinator on (08) 9599 4733 to confirm bed availability
  • Contact RGH on call Consultant to arrange acceptance and transfer of care
  • Inform PMH ED Coordinator of confirmed transfer
  • Provide all patient notes to Clerk to copy for transfer
  •  Complete EBM

Nursing

  • Arrange appropriate transport (e.g. private, transport ambulance, paramedic ambulance)
  • Contact RGH Children’s Ward Nursing coordinator on (08) 9599 4733 to give nursing handover and provide ETA 
Midland Health Campus
  Medical 
  • Contact MHC Paediatric Ward Coordinator on (08) 9462 4750 to confirm bed availability
  • Contact MHC on call Consultant to arrange acceptance and transfer of care
  • Inform PMH ED Coordinator of confirmed transfer
  • Provide all patient notes to Clerk to copy for transfer
  •  Complete EBM

Nursing

  • Arrange appropriate transport (e.g. private, transport ambulance, paramedic ambulance)
  • Contact MHC Paediatric Ward Coordinator on (08) 9462 4750 to give nursing handover and provide ETA
